Programs

Case Management

Services that assist participants in gaining access to needed waiver and other Wyoming Medicaid State plan services, as well as needed medical, social, educational, and other services, regardless of the funding source for the services to which access is gained. Case management includes ongoing monitoring of the provision of services included in the individual plan of care, and initiating and overseeing the process of assessment and reassessment of the participant's level of care and review of the individual plan of care.

Homemaking

Services consisting of general household activities (meal preparation exclusive of the cost of meals, and routine household care) provided by a trained homemaker, when the individual regularly responsible for these activities is temporarily absent or unable to manage the home and care for him or herself or others in the home.

In-Home Support

The provision of intermittent one-to-one habilitation services provided in the participant's home or the community to participants who reside with family, guardians, or indepently. Individuals receive skills training to increase independence related to their own health care, self-care, safety, and access and use of community services.

Personal Care

Assistance with eating, bathing, dressing, personal hygiene, activities of daily living. Personal Care services may include the preparation of meals, exlusive of the cost of the meals. When specified in the plan of care, this service may also include housekeeping chores such as bed making, dusting and vacuuming, which are essential to the health and welfare of the participant, rather than the participant's family.

Residential Habilitation Training

A Trainer provides participant specific, individually designed and coordinated training. He or she assists participants in acquiring, retaining, and improving the self-help, socialization, adaptive, and safety skills necessary to reside successfully in home and community based settings.

Respite

Respite is provided to a participant who is unable, unassisted, to care for himself or herself and the participant's primary caregiver is absent or in need of relief from furnishing such services on a short-term basis.
